Description
Anterior sectional anatomy centers on the adult female urinary bladder, with the urothelium and lamina propria lining the lumen and the detrusor muscle forming the thick intermediate wall. Just external to the muscularis, the subserosal layer (subserosa) appears as a thin sheet of loose connective tissue that separates the detrusor from the outer serosa where peritoneum reflects onto the bladder dome. Superiorly the bladder apex approaches the peritoneal cavity, while inferiorly the bladder neck narrows toward the urethra; the trigone sits posteroinferior within the lumen even in an anterior cross-section. Sparse vessels and adipose may be indicated within the subserosa, lying superficial to the detrusor but deep to the serosal surface. Emphasis on the subserosal connective tissue matters because it defines the plane between bladder wall proper and peritoneal covering, a distinction that guides both imaging interpretation and surgical dissection at the bladder dome. During cystotomy, partial cystectomy, or repair of iatrogenic injury, recognizing whether a defect is extraperitoneal (anterior wall and neck) versus intraperitoneal (at the superior surface) changes contamination risk and closure strategy. Peritoneal reflection and the subserosal plane also help explain why intraperitoneal bladder rupture after blunt trauma tends to involve the dome, where the serosa and its underlying subserosa are closely related.
