Description
Curving across the upper thorax, the superior border of the heart is formed primarily by the right atrium and left atrium, with the bases of the great vessels rising immediately superior to the atrial roof. From a left-sided viewpoint, the left atrium sits posteriorly while the right atrium contributes more anteriorly, and the superior vena cava enters the right atrium at the right superolateral aspect. The ascending aorta occupies a more central, anterior position, the pulmonary trunk lies anterolateral to it, and the left pulmonary artery courses leftward beneath the aortic arch region as it approaches the left lung hilum. This border matters when you are orienting mediastinal anatomy on imaging and in the operating room because small shifts in atrial size or great vessel contour change the silhouette and the available surgical corridor. Left atrial enlargement, classically from mitral valve disease, expands the posterior-superior cardiac contour and can alter the relationship to the left main bronchus and esophagus, a pattern that shows up on chest radiography, CT, and transesophageal echocardiography. Landmarks along the superior margin also guide cannulation strategies during cardiopulmonary bypass and help frame the approach to the interatrial groove in atrial septal defect repair.
