Seen from the left lateral aspect, the superior border of the heart is defined by the superior portions of the right and left atria, with the atrial domes curving posterior to the anteriorly placed great vessel roots. With the vessels removed, the superior mediastinal continuity becomes clear: the venous pole lies more posterior and superior, while the arterial pole normally occupies a more anterior and central position. The left atrial roof sits posterior to the main bulk of the ventricles, and the left atrial appendage would be expected anterosuperiorly near the left atrioventricular groove even if not emphasized in this crop. Stripping away the aorta, pulmonary trunk, and caval veins isolates a teaching point many learners miss, the superior cardiac contour is often atrial rather than ventricular. That distinction matters when correlating physical and imaging findings, for example, left atrial enlargement from mitral stenosis or chronic atrial fibrillation alters the posterior-superior silhouette and can elevate the left main bronchus, while pericardial effusion tends to expand the cardiac outline more globally without preferential atrial doming.
