Description
Arising from the superolateral aspect of the left atrium, the left auricle (left atrial appendage, auricula sinistra) projects anteriorly and leftward while remaining visible from a posterior orientation because it wraps around the superior border of the atrium. Pectinate muscles create a corrugated external contour within the appendage, contrasting with the smoother atrial wall adjacent to the pulmonary venous inflow. Superior to the appendage lies the level of the left pulmonary artery and the reflection of the pericardium, while inferior and medial relationships trend toward the posterior left atrial wall and the atrioventricular groove. Orientation matters. Left atrial appendage anatomy carries specific clinical weight because this blind pouch is the dominant site of thrombus formation in nonvalvular atrial fibrillation, and subtle variation in its lobes and neck can change both embolic risk assessment and device selection. A posterior view also supports teaching of transesophageal echocardiography concepts, where the appendage can be partly obscured and must be interpreted relative to the left superior pulmonary vein and the posterior left atrial wall. Procedural planning for left atrial appendage occlusion and surgical ligation depends on clear mental mapping of this structure’s position at the left atrial roof and its relation to the pericardial reflections.
