Arising from the superolateral aspect of the left atrium, the left auricle (left atrial appendage, auricula sinistra) projects anteriorly and leftward while remaining visible from a posterior orientation because it wraps around the superior border of the atrium. Pectinate muscles create a corrugated external contour within the appendage, contrasting with the smoother atrial wall adjacent to the pulmonary venous inflow. Superior to the appendage lies the level of the left pulmonary artery and the reflection of the pericardium, while inferior and medial relationships trend toward the posterior left atrial wall and the atrioventricular groove. Orientation matters.
