Description
Arising from the anterosuperior aspect of the left atrium, the left auricle (left atrial appendage, auricula sinistra) projects anteriorly and slightly superiorly, draping toward the left side of the great arterial root. Its lumen is characterized by prominent pectinate muscles forming a dense, trabeculated surface that contrasts with the smoother left atrial body located medially and posteriorly. From a left-sided boundary representation, the appendage appears as a tapered, ear-shaped pouch with an ostium directed posteromedially into the atrium. No pulmonary veins, coronary vessels, or arterial trunks are included, keeping attention on the auricular wall architecture and margins. Clinically, the left atrial appendage is the dominant site of thrombus formation in nonvalvular atrial fibrillation, and its variable lobar anatomy influences both stroke risk and device selection for percutaneous occlusion. Pectinate muscle ridges can mimic thrombus on transesophageal echocardiography, so recognizing their organized, parallel relief helps in interpretation. For surgeons and electrophysiologists, the external contour and ostial orientation matter during appendage exclusion with clips or stapling and when planning ablation lines that traverse the left atrial roof and lateral wall. Small structure, big consequences.
