Description
Dominating the left-sided cardiac silhouette, the left ventricle forms the apex and most of the left (obtuse) margin, its thick myocardium evident even on an external surface view. Superior and posterior to it, the left atrium sits at the base of the heart, with the left auricle projecting anteriorly and laterally along the atrioventricular groove. The interventricular sulcus marks the anterior boundary with the right ventricle, while the coronary (atrioventricular) sulcus separates atrial from ventricular myocardium. Orientation is straightforward. Left-heart emphasis matters because it correlates directly with systemic afterload and the pathology that follows it, concentric left ventricular hypertrophy in long-standing hypertension and pressure overload lesions such as aortic stenosis. External morphology also supports operative planning and teaching around the left atrial appendage, a frequent source of thrombus in atrial fibrillation and a target for occlusion devices or surgical ligation, and the obtuse marginal coronary territory on the lateral left ventricle. Surface anatomy is the first map in the chest. Cardiology and gross anatomy faculty can use this plate to anchor chamber identification before introducing coronary circulation, valve planes, and the relationship of the left atrium to the pulmonary veins in a thoracic block.
