Description
Viewed from an inferior perspective within the thoracic cavity, the right main bronchus is traced as it diverges from the distal trachea at the carina and courses inferolaterally toward the right lung hilum. Its lumen appears wider and its segment is shorter than the left main bronchus, reflecting the typical adult asymmetry at the tracheobronchial bifurcation. Medially, the bronchus relates to the mediastinum, while laterally it approaches the visceral pleura and pulmonary parenchyma at the root of the lung. Orientation matters. This angle is the one clinicians mentally reconstruct during airway procedures and when interpreting bronchoscopic or CT findings around the carina. The more vertical, capacious right main bronchus explains why aspirated material and endobronchial foreign bodies more often lodge on the right, commonly in the bronchus intermedius and lower lobe bronchi. It also frames safe placement of an endotracheal tube, where inadvertent right mainstem intubation can collapse the left lung and overventilate the right.
