Description
Running inferiorly from the thoracic inlet, the intrathoracic trachea is seen in right lateral aspect with its C shaped tracheal cartilages stacked superior to inferior and bridged posteriorly by the membranous wall containing the trachealis muscle. Anterior and slightly rightward, the brachiocephalic trunk ascends toward its bifurcation while the superior vena cava descends vertically in the right mediastinum, forming the most immediate vascular neighbors along the trachea’s anterolateral margin. Posterior to the airway, the esophagus lies close against the tracheal membranous wall, an intimate relationship that becomes clearer on a lateral projection. Distally, the trachea approaches the carina at approximately the T4 to T5 level. Right sided lateral anatomy of the thoracic trachea matters because it is the corridor shared by airway instrumentation and mediastinal vascular structures that are vulnerable during tracheostomy extension, mediastinoscopy, and emergent airway management. This angle also supports teaching the clinical basis of tracheo-innominate fistula, a rare but catastrophic hemorrhage when a low tracheostomy or prolonged cuff pressure erodes into the brachiocephalic artery where it crosses anterior to the trachea, and it helps explain why right paratracheal masses or lymphadenopathy can narrow the airway while simultaneously compressing the superior vena cava.
