Seen from the left lateral aspect, the thoracic part of the trachea (pars thoracica tracheae) descends in the superior mediastinum from the thoracic inlet to the carina, where it bifurcates into the right and left main bronchi. The airway lies anterior to the esophagus and posterior to the great vessels, with the aortic arch sweeping superior and leftward over the tracheobronchial region and the left pulmonary artery coursing anterior to the left main bronchus. Inferiorly, the carina sits near the sternal angle region, marking the transition from a single intrathoracic tube to the bronchial tree. Short and clear. A left-sided profile matters because the aortopulmonary window and the relationship of the trachea to the aortic arch shape both endoscopic orientation and cross-sectional interpretation on CT. Subcarinal and left paratracheal lymph nodes commonly enlarge with bronchogenic carcinoma, lymphoma, or granulomatous disease, and this view clarifies why these nodes can narrow the airway near the carina. It also supports procedural planning for mediastinoscopy and endobronchial ultrasound, where millimeters of anterior-posterior depth determine safe access around vascular structures.
