Description
Viewed from an inferior perspective, the intrapulmonary bronchial tree is traced from the main bronchus into secondary (lobar) bronchi and on into segmental bronchi coursing within the lung parenchyma as tapering airways. Branches radiate laterally toward the costal surface and curve medially toward the mediastinal surface, with proximal lumens broader near the hilum and distal tubes narrowing as they approach the periphery. The basal segmental bronchi are emphasized in this orientation, aligning with the diaphragmatic surface of the lung. Spatial relationships read bottom up: inferior bronchial branches sit closest to the diaphragm, while more superior divisions recede toward the hilum. An inferior view clarifies why lower-lobe disease behaves the way it does. Aspirated material most often tracks into dependent segments, classically the posterior basal segments of the lower lobes in supine patients and the basal segments in upright patients, so mapping segmental bronchi from below helps correlate bedside findings with bronchoscopy and suctioning targets. It also supports surgical planning for segmentectomy, where correct identification of segmental bronchi (and their accompanying segmental arteries) determines resection margins and reduces postoperative atelectasis. A critical landmark. Common use cases include teaching bronchopulmonary segments in gross anatomy, respiratory blocks, and thoracic surgery modules, as well as illustrating bronchoscopy pathways in clinical skills manuals and journal figures discussing aspiration pneumonia, mucus plugging, or endobronchial obstruction.
