Description
Segmental bronchi arborize within each lung as intrapulmonary continuations of the right and left main bronchi, dividing into secondary (lobar) bronchi and then into tertiary (segmental) bronchi that track with pulmonary arteries toward the periphery. From an anterior orientation you would expect the tracheal bifurcation at the carina superiorly, the right superior lobar bronchus branching early and laterally, and the longer left main bronchus coursing inferolaterally beneath the aortic arch before giving rise to its lobar branches. More distal segmental tubes fan outward, with apical and anterior segments positioned superiorly, and basal segmental branches descending inferomedially toward the diaphragmatic surface. Bronchopulmonary segments form discrete, wedge-like territories. Segmental anatomy matters because the tertiary bronchi define bronchopulmonary segments, the smallest surgically resectable units of lung with segmental bronchovascular supply and intersegmental venous drainage. Surgeons rely on these planes during segmentectomy for early-stage peripheral non small cell lung carcinoma and for localized bronchiectasis, aiming to spare functioning parenchyma while maintaining margins and preserving adjacent segments. Bronchoscopy, too, becomes more deliberate when you can map a mucus plug or bleeding source to a specific segmental bronchus.
