Description
Bronchial pathways course through the lung parenchyma as progressively branching tubes, beginning with the secondary (lobar) bronchi and continuing into segmental bronchi directed toward their bronchopulmonary segments. Proximally, the lobar bronchi arise from the main bronchus and pass laterally into the substance of the lung, then divide distally into smaller-caliber segmental branches that track alongside segmental pulmonary arteries. Airway caliber tapers with each generation. The parenchyma surrounds and supports these conducting structures. Segmental bronchial anatomy matters because it maps directly to bronchopulmonary segments, the surgical and radiologic units used for segmentectomy and for localizing obstruction, hemorrhage, and infection. Mucus plugging or an aspirated foreign body commonly lodges in more distal bronchi, and understanding the branching pattern helps explain why atelectasis and post-obstructive pneumonia respect segmental boundaries. Bronchiectasis also follows these pathways, with cylindrical or saccular dilation best interpreted when you can orient lobar versus segmental bronchi and their relationship to adjacent vessels.
