Description
Intrapulmonary bronchial divisions branch from the main bronchus after it enters the lung at the hilum, continuing as lobar (secondary) bronchi and then segmental (tertiary) bronchi that distribute air through progressively smaller conducting tubes. Lobar bronchi course within each lobe, while segmental bronchi run more peripherally and inferiorly toward the bronchopulmonary segments, typically accompanied by segmental branches of the pulmonary artery in a close, parallel relationship. Distal to these larger tubes, the airway caliber narrows into smaller bronchi and bronchioles as the tree fans laterally toward the pleural surface. Teaching intrapulmonary bronchi by division level matters because these branches map directly onto surgical and radiologic units, not just airway anatomy. Segmental bronchi define bronchopulmonary segments, which is why a segmentectomy can remove diseased tissue with margins while preserving adjacent segments, provided the segmental bronchus and its artery are identified and controlled. This hierarchy also underpins bronchoscopy and airway obstruction patterns: a mucus plug or aspirated foreign body in a segmental bronchus produces segmental atelectasis, while more proximal involvement collapses an entire lobe. Pulmonary anatomy courses, thoracic surgery teaching files, and radiology texts commonly need a clear depiction of lobar versus segmental bronchi to pair with CT bronchograms, bronchoscopic orientation, and operative planning for lobectomy or segmentectomy.
