Viewed from below, the posterior lateral segment of the right hepatic lobe (Couinaud segment VII) occupies the posterosuperior part of the liver but presents on the diaphragmatic aspect near the posterolateral margin when the organ is rotated into an inferior perspective. Segment VII lies lateral to the right hepatic vein and posterior to segment VI, with the bare area abutting the diaphragm and the peritoneal reflections of the coronary and right triangular ligaments marking its superior and lateral attachments. Medially, the posterior aspect of the porta hepatis and the groove for the inferior vena cava define key relationships on the visceral surface. Deep to this region, the segmental portal pedicle for VII branches from the right portal vein. Orientation to segment VII matters because it is a frequent site for hepatocellular carcinoma and colorectal liver metastases, yet it remains one of the more difficult segments to access for limited resection or ablation due to its proximity to the right hepatic vein and the retrohepatic inferior vena cava. Surgical planning for a posterior sectionectomy (segments VI and VII) or an isolated segment VII segmentectomy depends on clear appreciation of the intersegmental planes that follow hepatic venous drainage rather than external lobar contours. Bleeding control. This inferior perspective also supports teaching the difference between anatomic lobes (right, left, caudate, quadrate by surface landmarks) and functional segments based on portal inflow, biliary drainage, and hepatic venous outflow.
