Description
Seen from a superior plane, the cranial (diaphragmatic) surface of the liver is oriented toward the viewer, with emphasis on the posterior lateral portion of the right hepatic lobe (lobus hepatis dexter). The convex hepatic dome occupies a posterolateral position relative to the falciform ligament and the more anteromedial left lobe, while the hepatic margin curves inferiorly away from the costal surface. Posteriorly, the contour implies proximity to the bare area where hepatic parenchyma abuts the diaphragm, and laterally it approaches the region that overlies the right kidney and adrenal in the subhepatic space. For segmental anatomy, this angle supports teaching the relationship of the right posterior sector (Couinaud segments VI and VII) to the hepatic veins and to the posterosuperior dome of the liver, a region that is frequently underestimated when planning access. Resection or ablation of lesions in segment VII often challenges laparoscopic exposure because the target sits high under the diaphragm, near the right hepatic vein as it drains to the inferior vena cava. Orientation from above clarifies why intercostal or transthoracic ultrasound-guided approaches are sometimes chosen for posterosuperior tumors, and why mobilization of the right lobe can be required to control bleeding from short hepatic veins. Surgical atlases, hepatobiliary oncology manuscripts, and anatomy curricula that teach Couinaud segmentation will benefit from this focused superior perspective on the male liver’s right posterior lateral region, where surface topography must be translated into operative landmarks.
