Description
Positioned on the right hepatic lobe (lobus hepatis dexter), the posterior lateral segment (Couinaud segment VII) occupies a posterosuperior corner of the liver, immediately inferior to the diaphragm and lateral to the retrohepatic inferior vena cava. From a posterior surface orientation, segment VII sits superior to the inferior right lobe parenchyma (segment VI) and posterior to the more anterior right anterior sector (segments V and VIII). Its deep parenchyma relates medially to the hepatic veins as they converge toward the caval confluence, while its lateral contour follows the convex diaphragmatic surface. Relationships matter here. Couinaud segment VII carries practical implications for both imaging and operative planning because its portal pedicles and venous outflow can be obscured on anterior views, yet they determine whether a lesion is resectable by an anatomic posterior sectionectomy versus a nonanatomic wedge. Radiologists localize tumors, abscesses, or metastases in segment VII by tracking the right hepatic vein and the plane of the portal scissurae, and surgeons often approach this segment with right lobe mobilization, control of short hepatic veins, and careful work near the cava and diaphragm. Segment VII is also a common site where subphrenic collections or post-hepatectomy bilomas can hide. Use this organizational map in hepatobiliary surgery atlases, radiology teaching files (Couinaud segmentation on CT and MRI), and anatomy curricula that cover functional liver anatomy beyond classic lobar descriptions.
