Description

Seen from an inferior (basal) perspective, the right lung’s middle lobe (lobus medius) occupies the anteromedial portion of the lung base, bordered superiorly by the horizontal fissure and posteriorly and inferiorly by the oblique fissure. Its visceral pleura curves toward the mediastinal surface, where the cardiac impression lies medial to the lobe’s anterior margin. Lateral to the middle lobe, the costal surface rounds onto the diaphragmatic surface, emphasizing how this secondary (intermediate) lobe wedges between the larger superior and inferior lobes. An inferior view makes the geometry of the fissures obvious, and that matters when you are teaching lobar anatomy or correlating it with imaging and bronchoscopy reports. Middle lobe pathology often presents as middle lobe syndrome, with recurrent atelectasis or infection related to the long, narrow right middle lobe bronchus and its angle from the bronchus intermedius, a pattern that can be mistaken for lower lobe disease if fissural boundaries are not kept straight.
