Description
Anteriorly, the right lung is presented with the middle lobe (lobus medius pulmonis dextri) picked out in color against the surrounding superior and inferior lobes. The horizontal fissure (fissura horizontalis) marks the superior border of the middle lobe, while the oblique fissure (fissura obliqua) defines its posterior and inferior boundary as it slopes inferomedially toward the diaphragmatic surface. Medially, the highlighted lobe wraps toward the hilum, where the relationship to the right cardiac impression is implied even when the mediastinal surface is not fully exposed. Clinically, isolating the right middle lobe from an anterior access point maps directly to the problem of right middle lobe syndrome, where recurrent atelectasis or infection reflects its long, relatively narrow bronchus and drainage challenges. This perspective also supports planning and explanation of a middle lobectomy through an anterolateral thoracotomy or VATS ports, since the surgeon works along the fissures and must anticipate the middle lobe vein’s course toward the left atrium.
