Precise Anatomy of the Middle Lobe Region of the Right Lung

Triangular lobus medius of the right lung appears in profile, bounded superiorly by the horizontal fissure and inferiorly by the oblique fissure as it wraps anteroinferiorly along the costal surface. Anteriorly, the middle lobe projects toward the sternocostal region, while posteriorly it tapers and yields quickly to the larger lobus inferior. Medially, the mediastinal surface faces the pericardium and hilum, where the right pulmonary artery, superior and inferior pulmonary veins, and the right main bronchus relate to the lobar bronchi and vessels supplying the middle lobe. Middle lobe anatomy matters because its bronchus (B4 and B5 segments, lateral and medial) branches at a shallow angle from the bronchus intermedius, a configuration that predisposes to mucus plugging and recurrent atelectasis in so-called middle lobe syndrome. Radiologists and bronchoscopists rely on the fissural boundaries and the anteromedial position of this lobe when localizing airspace disease that can be subtle on frontal chest radiographs but clearer on lateral views and CT. Surgical planning also depends on these relationships, since a right middle lobectomy requires secure control of the middle lobe vein and artery while preserving the venous drainage of the upper and lower lobes and respecting incomplete fissures that can increase postoperative air leak. Pulmonary and thoracic anatomy courses use this view to teach lobar orientation, fissure anatomy, and the difference between costal and mediastinal surfaces in the right lung.
