Description
Crossing the superior mediastinum, the left brachiocephalic vein (vena brachiocephalica sinistra, innominate vein) runs obliquely from the left venous angle toward the midline to join the right brachiocephalic vein and form the superior vena cava. Its course sits posterior to the manubrium and anterior to the great arterial trunks, with the vein trending from superolateral to inferomedial as it approaches the cavoatrial inflow. Superiorly, it lies inferior to the left sternoclavicular region and deep to the anterior thoracic wall. Clean isolation keeps attention on the caliber and direction of flow. This anterior presentation matters because the left brachiocephalic vein is the usual conduit traversed by catheters placed from the left internal jugular or left subclavian approach before they enter the superior vena cava. Left sided access is more prone to malposition or difficulty when the vein is narrowed or compressed between the sternum and underlying arterial structures, and it is a key landmark during mediastinal surgery and retrosternal dissection. Short. The long oblique segment also explains why left sided central venous lines can kink at the junction with the superior vena cava and why pacemaker leads may meet resistance.
