Description
Seen from the left lateral aspect, the left surface of the heart is dominated by the left ventricle, with the left atrium forming a smaller posterosuperior component. The obtuse (left) margin runs from the apex inferiorly toward the base superiorly, and the cardiac apex projects inferolaterally. Anteriorly, the anterior interventricular sulcus approaches the apex, while posteriorly the atrioventricular (coronary) sulcus separates atrial from ventricular myocardium. No great vessels are attached, leaving the ventricular contour and sulcal landmarks unobstructed. Left surface anatomy matters when you need to correlate external cardiac topography with chamber enlargement and surgical access. Left ventricular hypertrophy and dilated cardiomyopathy commonly broaden this lateral contour, a change that can be appreciated on imaging and at operation, and it alters the relationship between the apex and the left thoracic wall. Cardiology and gross anatomy courses use this view to teach which chamber forms the left heart border and where the apex beat arises clinically in the left fifth intercostal space at the midclavicular line.
