Description
Left ventricular endocardium is presented from a lateral perspective, forming a smooth, pale lining over the thicker left ventricular myocardium deep to it. Along the atrioventricular junction, the endocardial surface continues onto the mitral valve leaflets, and the layer remains contiguous into the left atrium superiorly. Trabeculae carneae and papillary muscle contours may be appreciated as subtle relief beneath the lining, with the chamber lumen lying medial to the ventricular free wall. Closely opposed layers, with no intervening plane. This orientation matters when teaching how endocardial pathology tracks chamber geometry and valve anatomy rather than the epicardial coronary distribution. Endocarditis on the left side most often involves the mitral or aortic valve and can extend onto adjacent endocardium, creating vegetations, leaflet perforation, and regurgitation that correlate with where blood flow strikes the lining. The same surface is the substrate for mural thrombus formation after an anterior myocardial infarction, where akinetic myocardium alters shear at the endocardium and promotes clot along the ventricular wall.
