Description
Along the inferior (visceral) surface of the right lobe of the liver, the colic impression appears as a shallow concavity created by the adjacent right colic flexure and proximal transverse colon. It lies inferior and lateral to the porta hepatis and gallbladder fossa, and it sits lateral to the duodenal impression, with the right kidney impression positioned more posteriorly. Peritoneal covering remains intact across these contact areas, so the “groove” is a topographic marking rather than a true fissure. Appreciating the colic impression helps orient the undersurface anatomy during hepatobiliary surgery and cross-sectional correlation. During laparoscopic cholecystectomy or right hepatectomy mobilization, the hepatic flexure can tether the liver via peritoneal reflections, and colon distension can alter the relationship between the gallbladder fossa and adjacent bowel. A small but real hazard is inadvertent colonic injury when dense adhesions or inflammatory change obscure the normal plane near the hepatic flexure. Clear surface landmarks reduce guesswork. Use this illustration in gross anatomy teaching blocks on peritoneal relations of the upper abdomen, in surgical atlases discussing right subcostal or laparoscopic approaches to the gallbladder and hepatic hilum, and in radiology education when mapping visceral impressions to CT or MRI of the liver’s inferior surface. It also fits well in patient-education materials explaining why right upper quadrant symptoms can overlap between biliary and colonic sources.
