Description
Inferior to the right lobe of the liver, along the visceral surface, the impressio duodeni appears as a shallow concavity created by contact with the first and second parts of the duodenum. Medially it lies near the porta hepatis and the fossa for the gallbladder, while laterally it blends into the broader visceral contour of the right hepatic lobe. Adjacent impressions that may be implied in this profile include the colic impression inferiorly and the renal impression posteroinferiorly. Orientation matters. Teaching the duodenal impression helps ground the three dimensional relationships of the hepatoduodenal region: the descending duodenum sits anteromedial to the right kidney and inferior to the liver, with the hepatoduodenal ligament bridging to the first part of the duodenum at the porta hepatis. For surgeons, this surface anatomy supports the mental map used during a Kocher maneuver, cholecystectomy, or exploration for bleeding posterior to the first part of the duodenum where the gastroduodenal artery runs. Radiologists also correlate this interface when describing mass effect and displacement of the duodenum by right lobe lesions or gallbladder pathology on CT.
