Medial sagittal section through the uterus exposes the endometrium with its glandulae uterinae extending from the luminal surface into the underlying endometrial stroma. Coiled, tubular uterine glands course inferiorly toward the basal layer, their secretory passages lined by simple columnar epithelium that faces the uterine cavity medially. Deeper in the wall, the endometrium abuts the myometrium, creating a clear junction for orienting gland depth relative to the muscular uterine body. Orientation is straightforward. Gland morphology is one of the fastest ways to teach, and recognize, endometrial cycling: straight, narrow glands in the proliferative phase versus tortuous, saw-toothed glands with luminal secretions in the progesterone-dominant secretory phase. That distinction matters in fertility workups and in histopathology, where abnormal gland-to-stroma ratio and cytologic atypia raise concern for endometrial hyperplasia and carcinoma, and where chronic endometritis can be suggested by altered gland architecture and inflammatory infiltrates. A medial division view also mirrors how endometrial biopsy and curettage samples are interpreted, since fragments are often assessed for gland shape, epithelial features, and relationship to stromal tissue rather than for gross uterine landmarks.
