Description
Arising from the pulmonary trunk, the right pulmonary artery courses horizontally to the right hilum and is presented from a posterior perspective, emphasizing its short, large-caliber main stem compared with the left. From this vantage, the vessel sits anterior to the right main bronchus (the classic RALS relationship), with the bronchus and adjacent hilar connective tissue partially framing the artery as it approaches the lung root. Proximally, its posterior surface faces the vertebral column and posterior mediastinum, while distally it begins to taper as it divides toward lobar branches within the lung. Orientation is straightforward. Posterior visualization matters because the right pulmonary artery is a frequent point of reference during thoracic surgery and interventional planning, where its relationship to the bronchus and pulmonary veins governs safe hilar dissection. In right upper lobectomy or pneumonectomy, surgeons often encounter the artery in close quarters with the superior pulmonary vein and bronchus; misidentifying the vessel or its early branches can lead to brisk hemorrhage and difficult control. The large caliber and short main stem also explain why central pulmonary emboli can lodge here and why catheter trajectories from the main pulmonary artery may enter the right side quickly.
