Positioned at the posteromedial wall of the second part of the duodenum, the distal common bile duct courses inferiorly to the hepatopancreatic ampulla, where the inferior sphincter of the bile duct forms a muscular cuff around the terminal duct. Posteriorly, the duct lies close to the head of the pancreas, while the duodenal lumen sits anterior to the sphincteric complex at the major duodenal papilla. The biliary channel descends in a gentle curve, becoming more fixed as it approaches its intramural segment. Small but decisive anatomy.
