Description
Posterior to the first part of the duodenum, the supraduodenal part of the bile duct descends within the right free margin of the lesser omentum (hepatoduodenal ligament), with the portal vein positioned posterior to it and the proper hepatic artery lying anteromedial. Superiorly, this segment continues toward the porta hepatis and inferiorly it approaches the superior part of the duodenum before becoming retroduodenal. Medial relations bring it near the epiploic foramen, while the gallbladder neck and cystic duct would lie more lateral and superior, converging to form the common bile duct. Orientation matters. Surgeons care about this posterior perspective because the supraduodenal bile duct is the segment most often exposed during cholecystectomy when assessing the extrahepatic biliary tree and controlling bile duct injury, and it sits in the same tight corridor as the portal vein, a configuration that leaves little margin for blind clamping. In malignant obstruction at or below this level (for example, pancreatic head carcinoma), proximal ductal dilation is interpreted with these anatomic boundaries in mind, and endoscopic or operative decompression strategies depend on where the narrowing lies relative to the duodenum and hepatic hilum.
