Description
Posterior inspection of the extrahepatic biliary tract places the superior sphincter of the bile duct (superior choledochal sphincter) along the distal common bile duct, just proximal to the hepatopancreatic ampulla (ampulla of Vater) within the wall of the second part of the duodenum. Superiorly, the common hepatic duct and cystic duct converge to form the common bile duct, which then descends to meet the main pancreatic duct at the major duodenal papilla. Medially and slightly inferiorly, the ampullary region and the sphincter complex sit close to the pancreatic head. Small structure. Big consequences. Functionally, this segment of the sphincter of Oddi modulates bile delivery into the duodenum and helps prevent duodenobiliary reflux. Posterior orientation is a practical teaching angle because it clarifies how a distal common bile duct stone can lodge near the ampulla and trigger obstructive jaundice, cholangitis, or gallstone pancreatitis by impeding both bile and pancreatic juice outflow at a single narrow gateway. It also sets the stage for understanding manometric patterns in sphincter of Oddi dysfunction and why interventions at the major papilla carry a pancreatitis risk.
