Description
Posteriorly oriented anatomy of the extrahepatic biliary tree is centered on the retroduodenal part of the common bile duct (ductus choledochus) as it descends inferiorly behind the superior part of the duodenum. Superiorly, the duct continues from the supraduodenal segment and lies in close association with the head of the pancreas, trending toward the pancreatic portion before reaching the ampullary region. The duodenum forms the anterior relationship, while the posterior surface of the pancreatic head and adjacent retroperitoneal tissues define the deep field. Spatially, this segment sits right of midline and courses inferiorly toward the second part of the duodenum. A posterior view is the practical way to understand why inflammation or tumor in the pancreatic head can obstruct the distal bile duct, producing painless jaundice in carcinoma of the pancreatic head or fluctuating obstruction in chronic pancreatitis. Surgeons and interventionalists also care about this segment during a Kocher maneuver, when mobilization of the duodenum exposes retroduodenal structures, and during biliary decompression planning, where the distal duct’s relationship to the duodenum and pancreas predicts access and risk.
