Description
Viewed from the left lateral aspect, the cardiac silhouette is covered by the serosa of the epicardium (lamina visceralis of the serous pericardium), the smooth visceral layer that closely invests the myocardium and conforms to the contours of the left ventricle and left atrium. Superiorly, the visceral serosa reflects onto the great vessels at the pericardial reflections, continuing as the parietal layer of serous pericardium, while inferiorly it approaches the diaphragmatic surface where the fibrous pericardium anchors to the central tendon. The left margin and obtuse border are emphasized, with the epicardial surface forming the interface to the pericardial cavity. That surface anatomy matters when you are thinking about pericardial space disease and surgical planes, because the visceral pericardium is inseparable from the heart, while the parietal pericardium is separated from it by a thin film of serous fluid. Pericarditis and pericardial effusion change the mechanics of that low-friction interface, and tamponade physiology ultimately reflects rising intrapericardial pressure rather than epicardial pathology per se. In the operating room, the epicardial surface is also the access layer for coronary bypass targets and for epicardial lead placement, and a lateral view helps orient the left ventricular free wall where pericardiotomy and exposure are planned.
