Description
Seen from the left lateral aspect, the serosa layer of the epicardium (visceral pericardium) forms the glossy outer covering of the heart as it drapes over the left ventricle. Deep to this serous layer lies the ventricular myocardium, and, where the wall is opened or sectioned, the trabecular part of the left ventricle appears as an irregular muscular surface lining the cavity. Toward the base, the epicardium continues across the atrioventricular groove, where coronary vessels typically course within subepicardial fat, while the apex sits inferior and anterior relative to the left atrium. That relationship matters when teaching pericardial anatomy because the epicardium is not a separate sac but the visceral layer of the serous pericardium, tightly adherent to myocardium and continuous with the parietal layer at the roots of the great vessels. Surgeons and imagers rely on this distinction. Pericarditis, pericardial effusion, and tamponade involve the potential space between parietal and visceral layers, while epicardial access for ventricular tachycardia ablation requires navigating along the cardiac surface with constant awareness of the left coronary circulation coursing in the atrioventricular and interventricular grooves.
