Description
Opened along an anterior section, the stomach lumen is lined by prominent rugae (plicae gastricae) that run as irregular longitudinal folds from the fundus and body toward the antrum. These mucosal wrinkles rise from the tunica mucosa and submucosa and sit internal to the muscularis externa, which forms the thicker wall deep to the fold pattern. Superiorly the rugae converge toward the cardia, while inferiorly they flatten as they approach the pyloric region. Rugal architecture matters because it changes with distension and with disease, and students often mistake normal mucosal folding for pathology on endoscopy or contrast studies. When the stomach is underdistended, thickened rugae can mimic infiltrative processes, while true hypertrophic gastropathy (for example Ménétrier disease) produces enlarged folds that persist despite adequate insufflation and may correlate with protein-losing gastropathy. A clean sectioned view helps anchor the concept that these folds are primarily mucosa and submucosa, distinct from the fixed external landmarks of the greater and lesser curvatures. Use this illustration in gross anatomy and GI physiology teaching to explain how rugae permit gastric expansion, and in radiology or endoscopy-oriented materials to frame normal fold patterns versus fold thickening in gastritis, edema, lymphoma, or linitis plastica.
