Description
Arcing along the anterior aspect of the male colon, the plicae semilunares coli form repeated crescentic mucosal folds that project into the lumen and demarcate the haustra. Each fold spans partway across the internal circumference, producing a scalloped contour between the taeniae coli that is best appreciated when the bowel is opened and viewed from the front. The segmental pattern implies the usual colonic calibers, with a wider transverse colon superiorly and a narrowing caliber toward the sigmoid colon inferiorly and to the left. Mucosa meets mechanics here. Clinically, these semilunar folds explain why the colonic lumen is not a smooth tube during colonoscopy, and why haustral boundaries can create partial visual barriers that hide flat lesions along the proximal edges of folds. The same fold pattern underlies normal haustral markings on plain radiographs and CT, a key distinction from small bowel valvulae conniventes which traverse the full lumen. Loss of haustration, classically described as a lead pipe colon, is a useful imaging and teaching correlate in long standing ulcerative colitis, while altered fold prominence can be seen with spasm or distension. Faculty can drop this into gastrointestinal anatomy and endoscopy teaching to link gross morphology of the large intestine to what trainees actually navigate on scope. It also fits cleanly into radiology primers on bowel pattern recognition and into surgical atlases discussing colonic mobilization where the external taeniae and internal haustra correspond.
