Description
Anterior sectioning through the gastric wall brings the serous coat (visceral peritoneum) to the surface and places the subserosa directly deep to it as a thin band of loose connective tissue. Deeper layers follow in order, with the tunica muscularis (outer longitudinal, middle circular, and, where present, inner oblique fibers) lying internal to the subserosa and external to the submucosa and rugae-forming mucosa. Along the outer curvature, the subserosa blends laterally toward the peritoneal reflections that continue into the greater omentum, while medially it approaches the lesser curvature where peritoneum and neurovascular structures converge. Subserosal tissue matters because it is the plane in which the stomach interfaces with the peritoneal cavity and adjacent ligaments, and it often becomes the surgical and pathologic “boundary layer” encountered during mobilization. Gastric carcinoma staging hinges on depth of invasion, and extension into the subserosa (T3) correlates with serosal involvement risk and peritoneal dissemination, a distinction that guides operative planning and adjuvant therapy decisions. Surgeons also meet this layer during laparoscopic wedge resection or distal gastrectomy, where staying superficial to the muscularis propria helps avoid entering the lumen while still achieving an oncologically sound margin.
