Description
Along the lateral aspect of the left heart, the epicardium (visceral layer of serous pericardium) is presented with emphasis on its subserosa, the subepicardial connective tissue layer that lies deep to the mesothelium and superficial to the myocardium. From this peripheral orientation, you would expect to see the subepicardial tela thickening toward the atrioventricular (coronary) sulcus and tapering over the lateral wall of the left ventricle, with adipose tissue interposed between the epicardial surface and underlying cardiac muscle. Coronary vessels course within this plane, running parallel to the surface and separating from the myocardium by only a thin connective tissue interface. That relationship matters in the operating room and at the microscope. Subepicardial fat over the left ventricle and along the left atrioventricular groove can obscure the left circumflex artery and its obtuse marginal branches, a practical issue during coronary artery bypass grafting and during dissection for teaching labs. It also explains why epicardial coronary arteries are prone to iatrogenic injury during pericardiotomy or epicardial lead placement, where the dissection plane can drift from the pericardial sac onto the visceral pericardium and into the subserosa.
