Description
Seen from a left lateral peripheral perspective, the epicardium is sectioned to emphasize its subserosa (subepicardial tela), the connective tissue plane that lies deep to the mesothelium and superficial to the myocardium. Variable adipose tissue occupies this layer and tracks along the atrioventricular and interventricular grooves, where the coronary vessels and accompanying nerves course within the subepicardial connective tissue. The left ventricular free wall sits inferior and posterior to the left atrium, and the pericardial surface remains the most superficial boundary in the field. A thin plane, but clinically busy. Subepicardial fat is not just a background texture, it is the working corridor for the left coronary circulation and a key reason epicardial dissection behaves differently from myocardial dissection. This lateral view supports teaching of why coronary artery bypass grafting and coronary exposure via a left thoracotomy or minimally invasive approaches must respect the epicardial-subepicardial plane to avoid shearing small epicardial branches and provoking diffuse oozing.
