Description
Seen in lateral profile on the left side of the heart, the epicardium (visceral layer of serous pericardium) forms the outermost surface, with the subserosa (subepicardial tela) immediately deep to it and superficial to the dense myocardium of the left ventricle. The subserosal connective tissue layer is positioned between the mesothelium and cardiac muscle, following the curvature of the ventricular free wall from base toward apex. Small-caliber coronary branches and accompanying veins would logically course within this subepicardial plane, embedded in variable adipose, before penetrating the myocardium. Spatially, the serosal surface remains superficial and lateral, while the myocardium lies deeper and more medial. Understanding the subepicardial layer matters when you need to explain how the coronary vasculature and cardiac nerves reach the myocardium and why pericardial and epicardial disease can extend into adjacent fat and connective tissue. This is the dissection plane surgeons encounter during off-pump coronary artery bypass grafting and during exposure of the left anterior descending or circumflex territories, where traction, cautery, or inflammation can injure subepicardial vessels and ganglionated plexi.
