Description
Running across the dorsum of the foot, the dorsal venous arch forms a transverse venous arcade at the level of the metatarsal bases, receiving dorsal digital and metatarsal veins from the toes and forefoot. Medially, the arch continues proximally as the great saphenous vein as it passes anterior to the medial malleolus, while laterally it drains toward the small saphenous vein coursing posterior to the lateral malleolus. An elevated oblique perspective also allows the superficial veins to be related to the underlying extensor tendons over the midfoot and the bony landmarks of the tibia, fibula, and tarsal complex. Good surface anatomy. This configuration matters when you need reliable superficial access or when edema and thrombosis change the exam. Cannulation on the dorsum of the foot commonly targets tributaries of the dorsal venous arch, and placement becomes less predictable when the arch is displaced by swelling or when superficial thrombophlebitis tracks along the great saphenous trunk. For varicose vein assessment and saphenous mapping, the relationship to the malleoli is a practical landmark, and this view makes the medial and lateral drainage pathways immediately legible. Educators can drop this into gross anatomy and vascular anatomy teaching to anchor the saphenous system in palpable surface landmarks, or into nursing and paramedic training modules on peripheral IV access sites and pitfalls.
