Description
Spanning the dorsum of the foot, the dorsal venous arch forms a curved confluence of superficial veins immediately superficial to the extensor tendons and dorsal fascia. Medially, the arch continues proximally as the great saphenous vein, passing anterior to the medial malleolus, while laterally it drains into the small saphenous vein coursing posterior to the lateral malleolus. Superficial tributary veins from the dorsal metatarsal region and digits converge into the arch, with the metatarsals and phalanges providing the deep osseous framework beneath. A dorsal view of this venous pattern matters because it mirrors the common sites and trajectories used for peripheral venous access on the foot when upper-limb access is limited, and it clarifies why cannulation near the malleoli can be painful or unstable when the catheter crosses a mobile joint line. Venous return from the dorsum also becomes clinically obvious in edema, chronic venous insufficiency, and after saphenous vein harvest, when collateral pathways across the dorsal venous arch may dilate. Small valves and variable tributaries explain why reflux and superficial thrombophlebitis can track along the great saphenous side or the lateral marginal vein into the small saphenous system.
