Description
Centered along the cranial vault, the superior sagittal sinus runs in the attached margin of the falx cerebri, immediately deep to the calvaria at the midline. A cut section through the skull demonstrates its characteristic triangular lumen, bounded by the periosteal and meningeal layers of dura mater, with the apex directed inferiorly toward the falx and the broad base against the inner table. Laterally, cortical bridging veins course from the cerebral convexities toward the sinus, crossing the subdural space to enter the dural wall. Adjacent parasagittal cerebral cortex and its sulci sit inferior to the dural reflections. This cross-sectional profile matters because the superior sagittal sinus is a common site of dural venous sinus thrombosis and a key landmark for neurosurgical corridors near the midline. Injury to bridging veins at their entry points can produce subdural hematoma, and their relationship to the sinus helps explain why hemorrhage often tracks over the cerebral convexity rather than into the interhemispheric fissure. Midline craniotomy planning also hinges on the sinus position relative to the sagittal suture, and on the presence of dural venous lakes and arachnoid granulations along the sinus wall.
