Description

Seen in cut section of the adult male skull, the cavernous sinus lies lateral to the sella turcica and body of the sphenoid, forming a trabeculated dural venous space on either side of the hypophyseal fossa. Anteriorly it relates to the superior orbital fissure and posteriorly to the petrous apex, while its medial wall abuts the pituitary gland and its lateral wall is formed by dura along the middle cranial fossa. Blue venous channels extend along the inner table to the superior sagittal sinus at the midline, then sweep posterolaterally toward the transverse sinus at the occipital attachment of the tentorium cerebelli. Bony landmarks of the anterior, middle, and posterior cranial fossae frame the venous anatomy. Cavernous sinus anatomy matters because it is the only dural venous sinus that contains an artery and multiple cranial nerves within or immediately adjacent to its walls. Internal carotid artery (cavernous segment) and abducens nerve (CN VI) course through the sinus proper, while oculomotor (CN III), trochlear (CN IV), and the ophthalmic and maxillary divisions of the trigeminal nerve (V1, V2) run in the lateral dural wall, a layout that explains painful ophthalmoplegia in cavernous sinus thrombosis and the bruit, chemosis, and proptosis of a carotid cavernous fistula. Superior and inferior petrosal sinuses provide the main posterior egress toward the sigmoid sinus and internal jugular vein. Small spaces, high stakes.
