Description
Centered within the posterior cranial fossa, the straight sinus (sinus rectus) runs in the midline at the dural junction where the falx cerebri meets the tentorium cerebelli. A superior internal view of the calvaria frames the dural venous system in situ, with the superior sagittal sinus tracking anteroposteriorly along the attached margin of the falx and the transverse sinuses extending laterally from the confluence of sinuses near the internal occipital protuberance. From each transverse sinus, the sigmoid sinus curves inferolaterally toward the jugular foramen, while smaller meningeal venous channels radiate across the endocranial surface to join the major sinuses. Parietal and occipital bones form the bony margins of this cut skull section, keeping the relationships between dura and inner table explicit. Straight sinus anatomy matters because it is the key conduit between the deep cerebral venous system and the posterior dural sinuses, receiving the inferior sagittal sinus via the great cerebral vein (vein of Galen) before draining toward the confluence. Small shifts in its caliber or patency have outsized clinical impact. Dural venous sinus thrombosis involving the straight sinus can present with headache, altered mental status, and bilateral thalamic edema on imaging, and it changes endovascular access planning compared with isolated transverse or sigmoid sinus disease. Neuroanatomy and neuroradiology courses use this perspective to teach midline dural reflections and venous outflow patterns, and it fits cleanly into atlas plates comparing MR venography with gross anatomy. Neurosurgical and ENT publications can also cite it when discussing occipital craniotomy corridors, tentorial pathology, or posterior fossa approaches where venous sinus injury is a real operative risk.
