Description
Viewed laterally in a male thorax, the vertebral column runs superior to inferior through the thoracic and upper lumbar regions, with the sacrum anchoring the inferior segment. Curving anterolaterally from the thoracic vertebrae, the rib cage is shown in profile, with the twelfth ribs (costa XII) highlighted as the most inferior pair. Their posterior ends articulate with the body of T12 at the costovertebral joint, while their distal tips project anteriorly and inferiorly as short floating ribs, without an anterior attachment to the sternum or costal cartilages. No tubercle. This specific rib level matters because costa XII behaves differently than the typical thoracic ribs: it lacks a neck and tubercle, so it does not form a costotransverse joint, and its mobility at the thoracolumbar junction influences both injury patterns and procedural landmarks. Clinically, the 12th rib is a surface reference for renal anatomy and posterior approaches, and it is also a frequent site of fracture with flank trauma, where sharp distal segments can irritate adjacent soft tissues and contribute to focal pain syndromes (including slipping rib variants at the floating ribs). Its relationship to T12 also anchors teaching of transitional vertebrae and counting errors on radiographs.
