Description
Arcing posterolaterally from the thoracolumbar junction, the twelfth ribs (costae XII) appear as short, slender floating ribs articulating with the body and transverse process of T12 at the costovertebral region, with no anterior attachment to the sternum. Their proximal heads sit posterior and slightly superior to the distal tips, which project anterolaterally into the lateral abdominal wall and taper to free ends. Adjacent landmarks in this lateral thoracic cage view include the lower thoracic vertebrae, intervertebral discs, and the transition toward L1 and the sacrum inferiorly. Kyphosis of the thoracic spine sets the ribs in a posterior convex contour. Because rib 12 lacks a costal cartilage connection, its distal segment moves more independently than ribs 1 through 10 and becomes a practical surface landmark for the costovertebral angle. That matters in the clinic: tenderness or percussion pain at the angle just inferior to the twelfth rib often localizes renal pathology (pyelonephritis, perinephric inflammation), and the rib itself can be a limiting structure during posterior or flank approaches to the kidney. Fractures here behave differently too, with sharp free ends that can irritate adjacent soft tissues of the abdominal wall rather than destabilize the thoracic cage. Short and mobile.
