Description
Running longitudinally along the posterior thoracic spine, the semispinalis thoracis is highlighted deep to the erector spinae, spanning from transverse processes in the mid to lower thoracic region to spinous processes several segments superior. From a lateral perspective, its fibers can be read obliquely as they course superomedially toward the midline, lying medial to the angles of the ribs and posterior to the thoracic vertebral bodies. Inferiorly, the muscle sits just superior to the pelvis and adjacent to the thoracolumbar junction, where transversospinalis layers thicken. Bone landmarks dominate the field. Semispinalis thoracis matters because it is a key segmental extensor and contralateral rotator of the thoracic spine, and it provides stiffness across multiple motion segments during respiration and trunk loading. In posterior approaches to the thoracic spine (for example, laminectomy or instrumentation), this layer is encountered after reflecting the longissimus and iliocostalis, and its detachment from spinous processes contributes to postoperative paraspinal atrophy and persistent axial pain.
