Tibialis Anterior

Description

Dominating the anterior compartment of the cruris, the tibialis anterior runs immediately lateral to the tibial crest, its fleshy belly originating from the lateral condyle and proximal lateral surface of the tibia and tapering distally into a long tendon. Medial to it sits the subcutaneous anterior border of the tibia (shin), while the fibula lies lateral and slightly posterior, framing the interosseous region between the two bones. Distally, the tendon courses anterior to the ankle joint beneath the extensor retinacula toward its insertion on the medial cuneiform and base of the first metatarsal, with the tarsals, metatarsals, and phalanges providing the bony context for dorsiflexion mechanics. The knee and ankle articulations anchor the proximal and distal relationships. Clinically, this anterior view supports teaching of ankle dorsiflexion and inversion as a coupled action, and it clarifies why tibialis anterior is the dominant contour muscle of the shin in lean individuals. It also helps you localize the anterior tibial artery and deep fibular (peroneal) nerve, which run deep to tibialis anterior and become clinically relevant in anterior compartment syndrome and in traction or entrapment neuropathies that present with foot drop and dorsal first web space sensory loss.
