Description
Oriented along the anterolateral surface of the tibia, the tibialis anterior forms a thick fusiform belly in the proximal cruris and narrows into a tendon that passes anterior to the ankle joint toward the medial foot. The tibial shaft sits medial to the muscle, while the fibula remains lateral, framing the anterior compartment; distally, the tendon courses beneath the superior and inferior extensor retinacula before inserting on the medial cuneiform and base of the first metatarsal. Surrounding osseous landmarks commonly included in this composition include the distal femur, patella, tibial plateau, talus, calcaneus, and the tarsals, metatarsals, and phalanges, giving clear bony context for the ankle mortise and the medial longitudinal arch. For teaching dorsiflexion mechanics, few structures read as clearly as tibialis anterior because its line of pull runs anterior to the talocrural axis and slightly medial, producing dorsiflexion with an inversion moment that counterbalances the fibularis (peroneal) tendons. Clinically, this anatomy underpins anterior tibial tendon tendinopathy and traction-related pain along the tibial border (often grouped under medial tibial stress syndrome), and it also explains the gait pattern in deep fibular (peroneal) nerve palsy where loss of dorsiflexion produces foot drop and a compensatory high-stepping gait. The anterior compartment is also the classic setting for acute compartment syndrome after tibial fracture, where rising pressure threatens the deep fibular nerve and anterior tibial artery.
