Description

From a posterior, wide-angle perspective of the male thorax, the great vessels are centered on the mediastinum, with the thoracic aorta descending inferiorly and giving posterior intercostal branches that course laterally along the ribs. Systemic veins track back toward the heart as the azygos system ascends on the right side of the vertebral column, arching anteriorly to join the superior vena cava, while the hemiazygos and accessory hemiazygos cross from the left to drain into it. Superiorly, the aortic arch curves toward the left with proximal branches consistent with the brachiocephalic trunk, left common carotid, and left subclavian arteries, continuing into the upper limb as the axillary and brachial vessels. This posterior arrangement matters because it matches how you approach and interpret much of chest vascular anatomy in cross-sectional imaging and posterior surgical corridors. Azygos dilation in superior vena cava obstruction, collateralization with IVC interruption, and aberrant aortic arch branching all become easier to recognize when you keep the vertebral column and posterior intercostal spaces as fixed landmarks. Small variations matter. Use this artwork in gross anatomy and cardiothoracic modules to teach mediastinal compartments, the posterior intercostal neurovascular bundle relationship, and systemic venous return, or in radiology teaching files to pair with axial CT and MR angiography of the descending thoracic aorta and azygos vein.
