Description
Seen from an elevated oblique angle, the superficial veins of the scalp and face course over the frontal bone, parietal bones, zygomatic arch, maxilla, and mandible, then converge inferiorly toward the neck. The angular vein at the medial canthus continues as the facial vein, which descends posteroinferior to the body of the mandible and joins the internal jugular system via the common facial vein. Posterolaterally, the superficial temporal and posterior auricular veins drain toward the external jugular vein over the sternocleidomastoid, while deeper drainage tracks to the internal jugular vein within the carotid sheath adjacent to the cervical vertebrae. Venous anatomy in the head and neck matters because it is a low pressure, valveless, and variable network that can redirect flow when pathways are compressed, thrombosed, or surgically ligated. Infection in the “danger triangle” of the face can propagate retrograde from the angular vein into the superior ophthalmic vein and cavernous sinus, a classic route for cavernous sinus thrombosis. This view also clarifies clinically common access and injury zones: the external jugular vein crosses superficial to sternocleidomastoid and can be lacerated in penetrating neck trauma, while the internal jugular vein is the target for central venous catheterization using anatomic landmarks and ultrasound.
